Yes a tuning adapter is installed and on. The Cox technician reviewed the install and said everything looked fine. It is using a splitter and as I mentioned before the signal is very strong. Here are the diagnostics results you've identified above, my results are in red:

The two items I BOLDED are the issue.
Go to this page (login must be the primary email) and verify your CableCARD and Tuning Adapter RF MAC Addresses in the Drop-down, under TV Receivers - Select Your Hardware:
https://www.cox.com/resaccount/my-services.cox
CableCARD will be listed as
Make and Model: CSA800 (If yours is a CSA908 (PKM908), trade it in for a CSA800 (PKM800))
MAC Address:
Tuning Adapter will be listed as
Make and Model: CST1500
MAC Address:
CableCARD
Status: Not Staged (The Cox Headend does not recognize the CableCARD as Valid, It is not in their system properly)
SDV Server
Status: Unavailable (the Tuning Adapter is not communicating with the SDV Server)
You do not really need the splitter, try using the Tuning Adapter's RF pass-thru, This Could be a signal issue so check the following.
From the Tuning Adapter Diagnostics what are the following x values
Status Summary
> Next Page
Tuner: x dBmV (Should be as close to 0dBmV as possible, no less than -5 to -7dBmV)
FDC: x dBmV (Should be as close to 0dBmV as possible, no less than -5 to -7dBmV)
RDC: xx dBmV (Should be between 30 to 40dBmV, no greater than 50dBmV, over 55dBmV and I doubt it would work at all)
