Can you bypass the amp AND splitter for a day and see if the problem goes away? BTW, the amp is a power return one, so it's not good for noise on the upstream. Looks like you have noise across the whole range though.

It's a bidirectional amplifier and should be installed prior to any splitters.

IMO a modem shouldn't be on a amp if at all possible. However you are right, it should be before the splitter.
