How do I file a complaint for refund?

I have had 1000Mb internet service for almost a year and we have complained many times and had many Cox representatives out to fix the issue.  A month ago we found out we did not have the correct box on our pole.  I want to be refunded the money we paid for service we did not receive.  I cannot find on the website a place to do this.
