Equipment setup request

Trying to setup internet and when I go to activate is says, "hmm, we can't find your equipment setup request". How do I request equipment setup?

    Someone forgot to register your self-install.  Call or email Cox to initiate it.  Also, if you're installing a telephone modem (eMTA), mention it for their required Verification Process.
