Email spam filter rules not working

I have a filter rule to send SUBJECT "life insurance" to spam, and another to send FROM "life insurance" to spam. I am still getting emails with that subject in my inbox. I've looked it over carefully for spaces or any other shenanigan, and found nothing. Why is the rule not working?

    I have a filter rule to send SUBJECT "life insurance" to spam, and another to send FROM "life insurance" to spam. I am still getting emails with that subject in my inbox.

    Add the following Filter Rule to send email containing life insurance in the subject to Spam.
    Add condition: Subject > Contains life insurance
    Add action:      File into > Select folder > Spam > OK

    Do not enclose the Filter Rule text in quotes (life insurance vs "life insurance") unless you only want to send email to Spam if the filter text contained in the email subject is enclosed in quotes (see steps #4 thru #6 below).

    Review your Filter Rules to ensure a higher priority rule doesn't prevent this rule from running.

    Test steps  (beginning with the above Filter Rule)                                                      Email folder

    1. Sent myself an email with medical insurance in the subject.               Inbox
    2.  Sent myself an email with life insurance in the subject.                      Spam
    3. Sent myself an email with "life insurance" in the subject                     Spam
    4.  Enclosed filter text in quotes:  Subject > Contains "life insurance"
    5. Sent myself an email with life insurance in the subject.                       Inbox
    6. Sent myself an email with "life insurance" in the subject.                    Spam

    Note: The above rule can be modified to prevent email containing user determined selections in either the From or Subject columns from reaching any Webmail folder with the Discard action.
