Constant Connection Interruptions and massive Packet Loss

Over and over again over the last few weeks, I've had service interruptions that all follow the same pattern. My connection will come and go, seemingly halfway decent at times and completely useless at other times because of packet loss (anywhere up to 40-60% packet loss). Occasionally, the cox status page will show an outage in my area... other times, nothing. Either way, the phone tech support is completely useless, both swearing up and down my connection is fine, and then when asked to check the modem status/reset it admit that they cannot reach the modem.

When I check the modem logs, I'm seeing the same two errors over and over again, seeming to correspond to the times when I'm losing my connection:

1.) Received Response to Broadcast Maintenance Request, But no Unicast Maintenance opportunities received - T4 time out
2.) Unicast Ranging Received Abort Response - Re-initializing MAC

Looking at the connection data, most of it seems pretty normal, except for the downstream power which is anywhere from 10-14 dBmW on every channel.

Yes, I have tried resetting everything (repeatedly).

Yes, I have bypassed my router. I'm connected directly from my pc to the router using a brand new 3' length of ethernet cable.

No, I don't have any splitters between the cable modem and the wall. It's a direct connection and again, a brand new cable.
