Homelife is still listed on the statement you posted. I first thought Cox provided a free Internet modem because Homelife requires Internet, but Internet and equipment aren't included with Homelife.
When I downgraded my services back in April, I got rid of HomeLife, so naturally I returned the HomeLife hub and cameras...and none of it was on my account.
This from Cox's website:
"Homelife smart home service does not include equipment. You must purchase cameras from Cox to use the service".
Did you accidentally return your cameras to Cox? You may have just traded cameras for a modem.
Hmmm...guess I did, then. It was only one, so I'm still up a little 🙂
Not sure why HomeLife is being listed, been gone since I downgraded (no charges, either).

You should review your Cox monthly bills since April to determine if you received a credit for the returned equipment. If one was not received, you should locate documentation for the April transaction and contact Cox to request a credit for the camera and hub returned by mistake. It will probably be on your May bill.
The Cox website for "CANCELLING HOMELIFE SERVICE" includes the following:
- The Cox Homelife Touchscreen Control Panel must be returned to Cox to avoid being charged the associated non-return fee (if leased)
- All other equipment is owned by you and can be left in your home, discarded or taken with you.
Cox should have known the HomeLife camera was owned by you. They should have known the Touchscreen Control Panel (hub?) was owned by you when it wasn't included in the list of equipment on your account. Cox should have told you the equipment was your property and should not have accepted it without compensating you.
If you still have the Cox statements showing payment for the camera and hub, you should have that available as proof you purchased the equipment and to determine the approximate value of the equipment that was returned.

All I had was the hub (worst WiFi device I've ever owned) and the camera. Like I said, it's worth way less than the new modem, so I'm not going to worry about. I appreciate all the help/advice.
