password login

When I try to get on the internet.  I always get a box fo my user ID and password.  evem when I log in the information it keeps popping up.  How do i get rid of this.

Which program is generating the box?

Is it for your wifi?

Is it Outlook?

Any other program?

Considering the box can only be generated by a program running on your computer, it would be helpful to determine what it is for.

Can you take a screenshot of the box?  What is the text in the box?
