Does the spam blocker only work when nomorobo is activated?

I'm as confused as you are. Both the voicetools and the Cox website as well as this forum have many unresolved bugs that are still unresolved and probably will never be resolved.
Helpful Hint: When a moderator doesn't chime in for reassurance, yeah, it's glitchy, useless and broke.
Now I can't even access my settings.
You should just give up. First, Cox treats your wanted email as spam and instead of setting your spam preferences, Cox spams you with error messages.
Now I can't turn off the nomorobo it just stays turned on when I turn it off and I get the error saying that my settings can't be saved. Cox just verified that this issue is only happening using phones which I am using.
