Call Waiting not working properly

Call waiting is NOT sending a beep/tone when a call comes in while on a call.  The caller gets voice mail prompt to leave a message, I'm never aware there is an incoming call.

Checked on "Phone Tools" in account, Call Waiting is turned "On".
