packet loss (critical) - upstream (upload)

Hello,
I have upload packet loss, approximately > 3%. It seems to be based upon time of day---and over a long interval. It is intermittent. Incident #1: It began 1/4/2018 cleared up roughly around 1/19/2018. I had a Cox technician out, but he was a contractor and didn't have a lot of knowledge. He finally discovered a problem with the tap and called another tech with bucket truck...The tap was replaced outside near our house--but I still had intermittent issues.
I felt I was destined to put up with the errors ---feeling it was outside of my control. Later the issue cleared up (see diagrams).
Incident #2: Now (began 5/2/2018) , the upload packet loss errors are back (>3%). I use my Cox connection with corporate VPN/UCaaS service (required by my firm)---I was given a corporate SDWAN router (VeloCloud) to help with these issues---but the SDWAN error correction needs to have a decent connection (or more than 1 connection). The connection is pretty bad---as you can see by the graphs... Voice: while I can hear my fellow co-workers, my voice is garbled when speaking.
