Ceton infinitv6eth, cablecard, windows media center, no entitlement.

The card appears to have been paired, but I am only able to get channels 1005-1011

When I tune to any other channel media center says subscription required. if I check in the ceton diagnostic tool, its says no entitlement for the other channels.

Many calls to customer service have been made, this is the second card, performs the same as the first... 

Here is the output from the diagnostic tool...
