The Dynamic Range Violation, and Commanded Power entries are relatively unimportant. The profile assignment change is, as a understand it, a concern. Question is, what's the root cause? Can you show us the signal levels. A couple of other folks with more experience will probably chime in shortly.
+1 for showing the signal levels. My guess is it has something to do with the upstream OFDMA channel. Did you notice your upstream light went blue around the time the problem started? Last, edit out your MAC address for privacy reasons.
