Then someone must have logged in before and changed them from default. You can factory reset the modem/gateway but that will reset everything, including the network name and password. Cox tier 2 can remotely see signal levels but then your depending on them to tell the truth. Your call.
Figured out the login information. Attached are the not connected Upstream and Downstream and connected upstream and downstream

Yup, looks like a issue with the upstream dropping out. When the gateway resets it first locks onto one single channel on the downstream than upstream before it channel bonds. This is getting stuck locking on the upstream. I would try contacting Cox and show them this data and request a Cox technician(AKA in house) to investigate. If the line has been replaced my guess is the drop(what the line connects to at the pole) or further back in the infrastructure.
