This appears to be a misprint on the Cox site. The CM-16 is a DOCSIS 3.0 modem and the blue lights probably mean that multiple channels are bonded, not that it's running in DOCSIS 3.1 mode (which would be impossible since it is a DOCSIS 3.0 modem).
Even though this modem supports "up to 686 Mbps", it's generally not a good idea to use any DOCSIS device at more than 50% of its rated maximum speed. This is why it doesn't work on a 500 Mbps plan.

I will likely wait until Symmetrical 3.1 is available before investing in another modem.

FYI I'm not affiliated with Cox. Just a customer like everyone else, came here looking for answers and trying to help others where I can. FWIW, I've been very happy with the Arris SB8200 modem I got recently. It seems like a future-proof investment at least for the next few years.
