Our internet has been disconnecting for about 5-10 minutes almost every day. Also, the internet gets very slow multiple times, but does not go out. Here is the modem log. I blocked out the MAC addresses.

Yea, those are some serious errors. What are your signal levels? Almost looks like your downstream SNR is low or sometimes not broadcasting at all.
Also, can you describe the general layout of the cable wiring between the pole and your modem? Splitters? Amps? Etc

Yup, looks like I was right. Your downstream SNR (clarity) looks very low even though the signal strength is high. Should be above 36. That tells me something is interfering with the signal. You sure you don't have a signal amplifier? Perhaps a old one put in a long time ago, and some place out of sight? If that is the signal, give or take, from the street, then you need a technician.

I don't think there are any hidden amps because I got cox internet in May this year.
