I've been having problems with my cable tv for a while but haven't dealt with it because it is so intermittent. Here is a rundown of my setup:
When I had the cable installed, the installer said that the old Cox lines had been cut so he had to fix that to make it work. I have one coax line coming into my apartment. That is connected to a splitter with 3 outputs. One goes to the modem, one goes to my bedroom via a roughly 20 foot coax that is plugged directly into a tv(no cable box), and the 3rd one goes to my cable box with the big screen tv. I don't have dvr service. The cable box is plugged into a receiver via hdmi for picture and sound. I have a PS4 plugged into the same receiver via HDMI and have never had picture or sound problems when using it.
I haven't really had any problems with the internet. My girlfriend says she has problems with the internet but I'm pretty sure thats just her Mac.
Most of my problems have been with the cable box. It will reset itself randomly maybe once a day(usually when something important is on: Superbowl, Oscars, etc.). Sometimes when you change to a channel, it will be black with no sound, and then I will change the channel up, then back down and it will be fine. I have the common pixelation that others do. The hard drive makes a noise like it is starting to spin and then it makes a clicking noise and stops. I think that only happens when the box is turned off(I don't turn it off anymore and it seems to have stopped that noise). Sometimes the picture and sound will freeze and the cable box won't turn off via the power button. I have to unplug the power cable to get it to reset.
The cable signal in my bedroom has only had problems once. I was at work when it happened but my girlfriend said the tv in the living room was doing the same thing when it was going on and every channel had the problem. The link below is a video of it.

I was thinking of just taking the cable box into a store and asking for a new one but I thought I would try this first in case something else is causing the problem.

First, it sounds like the receiver may be having some hardware problems. What model is it?
When the receiver freezes (can't turn off) does the power button on the front do anything?
Last, what kind of splitter is it? If its equal signal to all outlets then you can check your modems signal levels for a general idea of the signal going to the receiver.

I would exchange the cable box just for the clicking noise. However, if you can live with the clicks, I'd remove the cable box from your home theater setup to test its signal and picture quality.
Connect the cable box directly to your TV. Do channels still blackout, pixelate and freeze? If so, exchange the cable box. If not, it could be an issue with your HDMI-to-A/V Receiver connection.
As Edge mentioned, where did you get your splitter?
- No cheap splitter from Radio Shack
- Rated for at least 1GHz. Satellite service uses 2.4GHz.
- Splitter with only the number of ports (outputs) you need. The more ports...the less signal to each port.
- Is there anything near your splitter or cable that would cause interference?
I feel exchanging your cable box would correct your discrepancies. Cox is suppose to bench-check their boxes before reissuing; however, they've issued me burnt boxes, boxes with completely inoperable HDMI ports and one with dead stink bugs.

By receiver I think you mean cable box, but my receiver is a Denon AVR987. The cable box is a Scientific Atlantic Explorer 8300HD.
When the cable box freezes, the power button on the front does nothing and I have to unplug the power to reset it.
The splitter says 5mhz - 1ghz and says 8db on two of the outputs and 4db on the third. The 4db I having going to my bedroom and the other two go to the cable box and modem.

I'll try connecting my cable box straight to my TV but I imagine it isn't a problem with my receiver since my PS4 is connected to it as well and I never have problems when using it.

It is a Scientific Atlantic Explorer 8300HD. I can usually turn it off with the power button, but when it freezes, it won't work and I have to unplug the power cable from the box.

Your splitter seems to be adequately rated (1GHz). I'd direct connect the cable box to your TV to test its signal and picture quality. If you still have the same problems, exchange the cable box. Ask for a new one.
