The service code and modem information is showing correctly for the speeds for that tier. Are aware of the type/model of the router you are connecting through?

Thanks for responding Ed H. I'm using a Linksys 802.11n showing speeds between 40 & 60 mbps with spikes up to 80 mbps. I'm betting this is my speed problem. Am I correct?

From the modem's logs it is only showing a 100Mbps connection using ethernet, which the modem ethernet port is up to 1000Mbps. I would try bypassing the router and connect direct to the modem to see if its ports are only 10/100 ethernet ports or by trying a CAT6 cable if the ethernet cable is older and not able to handle more then the 100Mbps speeds.
