Last year, when we moved temporarily to CA, we had low sound on the RCA output from the cable box into the TV. It took a long time to persuade Cox that the problem was not our TV. We had many Cox visits to our home, and we needed to borrow another TV to prove it was not our TV. Eventually a great Cox agent came to our home, pressed some keys on the Cox remote to get a hidden setting, changed the volume on RCA from low to high and the RCA sound became normal.
Now we are back in CA again and we have low sound again. Does anyone know how to get to that RCA specific setting?

I also had the DCX3400-M when I had cable. My guess would the audio output. You can access some of the options via the box's settings menu. Settings > Audio/Video > Audio Digital Output > Two Channel Only. This will broadcast PCM over the RCA. If this is already on, try Dolby Digital. For more info see here. Article doesn't show up under some zip codes. I had to pick Bonsall, CA on the top right of page for it to appear.
However, more settings can be accessed via the box's diagnostic menu. To get to those, with the box on, press power then press menu within 2 seconds. Then go to "Additional HDMI settings" and "Audio Output" and try changing it to "Pass Through". For more info see page 29-37 of manual.
PS. You mention in OP that you are using Red, White and Yellow? The yellow is for video, but RCA doesn't support HD, only up to 480p. Are you using HDMI for video and RCA for audio? If so, to what? Surround sound? TV? Etc
