1. When is going to be rolled out?

States: "Network upgrades in the markets have started and are expected to be completed by year end 2013."
Well, it's December 5, 2013. 27 days left of the year.
2. What can I do to prepare my router for the roll out?
3. Modems? Which cable modems are going to be supported? Currently have a Cisco DPC3010, which I hope is new enough to support IPv6.
Any information would be helpful.

Well 1 and 2 probably won't be answered anytime soon, because the timeline isn't set in stone yet.
As far as I know all D3 modems are IPv6 capatible.

1 and 2, weeeeellll.... There's only so much we can say. As far as I know, the infrastructure upgrades are done, or will be once FL and GA switch over in a few weeks. We've been standardizing a lot of stuff. but as for details, those are limited. I don't know when customer trials will start, and I'm not in any employee trials.
3, Anything on our current "recommended" list should be sufficient. http://bit.ly/CoxCompatibleModems. And I know the DPC3010 is one we've actually used for internal testing, so you're good.

Soooooo.....
There is hope by January 2014 you guys will be flipping the switch and turning on IPv6.

What I think Cox is doing is waiting as long as possible to transition to IPv6 on the customer side. They are probably waiting until their IPv4 stock reaches a certain rotation period. I have noticed Cox has started to change which IP's are used for where, so I think they might be changing stuff into their network first. This is just a guess though.
