I recently got a modem upgrade to a Cisco DPQ3212, and I've been experiencing a slow internet connection since it got activated for about a month now. I'm not really sure what is the optimal level for the Downstream/Upstream Channels, but it looks off to me.

Without splitter is better, but still not good. See here for signal levels. Do you have any other splitters between where it comes into the house and where it goes into the modem?
"Downstream power level (Receive): -15dbmV to +15dbmV
A value of -10 or worse indicates a poor downstream signal path. A tech would aim for a value close to the optimal 0 dBmV, but a good cable modem should be capable of working within the broader range of -10 to +10 dBmV, provided the downstream Signal to Noise Ratio remains good enough. Additionally all downstream channels should be within a +/-2db window with respect to each other."

No, I don't have any other splitters around. Though I wonder if the modem is the issue or my line, with my previous modem I wasn't having the slow speeds I'm having now.

If your previous modem was a DOCSIS 2.0 modem, then it may have had higher tolerances for weak signal. But a bad modem usually won't cause it to misread signal levels. If you have a straight line from pole to modem and getting those signal levels, then its time for a technician. Either call or wait for one of the moderators to respond.
