I am pretty sure I have a ground loop issue between my DVR box and my home theater pre-amp. I disconnected the incoming cable from the DVR and the speaker buzz goes away. Has anyone successfully used a ground loop isolator on their incoming cable line; and if so can you recommend a manufacturer and part number for the isolator?

It sounds like the internal volume setting on the box may be set too high. Have you tried locking the Cox remote's volume control to cable and turning the volume on the box down?

Try one of these?
Might cost more then the others but as stated here..;
"The model VRD-1FF is a capacitive based isolator which provides a high impedance barrier to AC power related ground currents,preventing this hum or buzz in sound systems or “hum bars” in video systems. The VRD-1FF was designed specifically to be a completely transparent path for video and cable modem signals. Unlike other low-cost CATV isolators available, the VRD-1FF has ruler-flat response and wide enough bandwidth to not cause an interruption of cable mode."
