Could you please have an AZ engineer look into why 1045 - TruTVHD is not mapped for CableCARDS in North Phoenix, AZ.
This channel says there is no signal detected, but the channels above and below work just fine.

TDsweetE,
going off of your local line up TruTV should come in for you whether it's standard or high definition. Does channel 45 work? Are you using the cable card in a TiVo, pc, television, or other device?

TruTV standard definition on Channel 45 works. TruTVHD on 1045 does not have data on it.
I have a cable TV tuner for my PC which uses the Cox CableCard. This channel worked previously on this system, and there have been no hardware changes since, so I'm confident that the issue is on the provider's end... and I can receive all the other channels that I subscribe to. Just not this one for some reason.

Now that I've pulled your account, I notice you don't have a tuning adapter. I think 1045 may be Switched Digital on one or two of the hubs, but Arizona is one of the areas where SDV is rolling out in waves.
Even if this particular channel is not SDV yet, it's a good idea to pick up a tuning adapter anyway. More and more channels will be SDV eventually.
