Decided to upgrade to premier internet tier yesterday but had nothing but issues. After processing the order the rep told me to reboot modem, restart computer and upon trying again speeds got worse. Today they called and said there were new codes that needed to be applied and I needed to reboot and when I did I lost internet and phone. After two hours on the phone I now have internet and phone back up but clock speeds the same and no ideas why no improvement. What am I missing here? They are opening a ticket to check this out but can't come until Tuesday, which is fine since I at least have working service again, but would really like to resolve this and get back to my weekend.

Important to note I am using a Cisco DQP3212 DOCSIS 3.0 Cable Modem

I have tried wired and wireless - the speeds are the same. My signal may be great but if no improvement to performance I may as well downgrade back down to preferred, instead paying extra for premium. Today's speedtest was 26 Mbps for Download and 9.33 Mbps for upload. I have tried from multiple devices with no material variance.

Have you tried bypassing your router? I ask because all your IPs are the same, which tells me the PC has never been connected direct to the modem for any test or it was for all those tests. Looks like something is constantly downloading with spikes of upload. Any of your PC running any kind of torrent programs?

Please disconnect your router and run the modem direct to the computer. That will effectively disable any other connections on your router if you are using one. Run a straight connection to the modem from your main in line at the house then direct to your computer, see if makes a difference.

I had run some of the test direct from Speettest.net, so those weren't reflected in what I posted.
Here is my udpate:
I swapped my Cisco E4200 router for the Netgear R6300 Smart Wi-fi router today and here are the new result (note this is running wireless and I am not hardwired into a port)
