I have a TIVO Premiere. Can I use it to get programs from Cox in HD, or do I have to have cable box or receiver?

If it receives HD, then it will receive Cox HD. You will need to rent a CableCARD from Cox (about $2/month), and get a "tuning adaptor" for SDV channels.
