I've hoped this problem would resolve itself in time or router restarts, but it still remains. The US (upload speed?) light for 1/2 is green and next to it for 1/4 is yellow (usually green). All other lights are green. I suspect it has to do with the upload channel 1 being zero... is this on my end or yours? Any help is always greatly appreciated.

What speeds are you getting for upload. Unless you have serious congestion, even 1 channel should be enough for 10Mbps. Also, I think the "Not locked modulation" means the channel is there just not being used, which shows that your area is not congested. However, a confirmation from a moderator might be prudent.
Also, what modem/gateway do you have?

I have a Netgear CMD31T
I am only getting about 8 Mbps, but I remember up until the light change on the front panel I was getting somewhere around 15.
Edit: It looks like I am getting only 6.23 Mbps download (and 58.64 Mbps download, 23 ms latency via cox speed check)

For starters, I'd suggest doing a simple test without the router for starters and see if you get the same results.

Plugged direct to to modem - light is still amber for DS 3/4, and upload speed is the same. Download speed is good.

The bonding state for the modem looks fine from here. If you're still seeing poor speeds connected directly to the modem it may be necessary for a technician to come out and troubleshoot further.
