Slow Upload Speed

I currently have the Internet premier plan. My upload speed is being much slower than it should be (only getting around 2mbps), this was an issue once before and a Cox support guy came out and said nothing was wrong with the lines and that he could do nothing. The issue then went away for around a month shortly after but it is now back. My download speed works perfectly and I have no issues with ping. I have tried directly plugging into the modem directly with ethernet and I still get the same upload speed. This is an issue on all devices connected to the network, they all get excellent download speeds but terrible upload speeds.

Router: Netgear Nighthawk, updated to latest firmware

Modem: Motorola Surfboard Sb6414, had been reset and restored to defaults multiple times to try to fix

Speedtest shows that speed, but when I actually upload a file it only gets around 2mbps at the most. Before I had the issue Speedtest would show 13-15 upload, so would testmy.net.

Testmy.net was done in Dallas.

Heres the speed if I try to upload a large file:

This is this speed everywhere I try to upload things, even to websites like youtube it will take 2+ hours to upload a video that would have taken 5-10 minutes before. I used to get always 10mbps or better, last time I had this issue I was getting around 4mbps so it has gotten worse.

I just ran another non flash based speed test that uploaded a 64MB file and I got 15ms latency, 900kbps, download was perfectly fine, it got 117Mbps on a 128MB file.

I tried swapping out the router with an Asus 68U and I got the exact same results, I don't have a spare Docsis 3.0 modem to see if that is the problem. 

Edit: Could this be the issue? I am only getting 2 upstream channels, but there are 8 downstream channels.

Edit 2: Contacted tech support about modem and Upload speed is now fixed.

Your upload signal level is a little high. Borderline, but still. Can you describe how the cable wire gets from the street to the cable modem?

Also, what can you post a tracert to testmy.net? Here is mine for comparsion.
