In addition to the MLB Extra Innings subscription, you'll have to pay more for the games in HD. Meaning, you'd have to rent something more expensive than a cable box. Didn't Cox tell you that when subscribing?
I discovered the hard way it's not a good value: Low Definition channels, blackouts, commercials and sometimes I couldn't watch the hometown network of my team.
Hi Estenv,
It's possible that the HD MLB Extra Innings channels are broadcast using the MPEG-4 format. If so, a cable receiver that supports MPEG-4 video is required. Our older HD receivers and DVRs are only capable of decoding MPEG-2. What model cable box are you using?
Even if I had a newer cable box, I couldn't watch HD MLB Extra Innings.
If you review the PDF of the Cox Channel Lineup, you'll see the HD channels for the MLB Extra Innings subscription are tagged with a Δ. For example, 1670-1678 NHL/MLB Pay Per View HD Δ.
What does the Δ mean?
I copied as follows: Δ Contour HD Channels, if included with your service level, require rental of a Contour receiver or Cox provided CableCARD together with a certified compatible CableCard retail device capable of tuning those channels.
Therefore, if you want the HD channels for the MLB Extra Innings subscription, you'll pay more as follows: - Contour Receiver Rental Fee - Cox Contour TV Service
@Bruce, you are correct, a Contour receiver is required in order to be able to view the higher range of HD PPV/Subscription channels. You also have access to use the MLB.tv app with your Extra Innings package. You can view HD streams through the app if you have a device or TV that has the MLB.tv app built-in. Otherwise, you can always look at it on a mobile device or computer.

not true you can buy a Tivo premiere or newer and get the Cox mpeg4 channels with cable card, but then you would also need a Tivo subscription.

At the very minimum, MLB.tv would add another 110 GB per month to your Internet Data Plan.
