Enter network password / cannot log into incoming mail server

I have 5 cox email addresses all w/the same settings.  

I cannot 'log into incoming email server' on one of them.  Outlook asks me for the password each time I send/receive. 

I *can* log into webmail using the password for this particular account.

I *can* send from this particular account. 

I have 'my server requires authentication' checked w/use log/password.  I've also tried use 'incoming mail login/password'.

Any thoughts?
