Are all Cable Cards equal?
I have a Pioneer TV with a Cable Card provided by Cox; it is an older one-way technology. All the digital conversion info I've seen says that a mini box or tuning adapter or set-top box is not required if customer has a cable card device. Somehow this does not seem to apply in my case....I've re-scanned but the tuner no longer picks up channels which have gone all digital, and they do not appear anywhere else in the channels received. So, is it possible that I need an updated cable card that Cox hasn't told me about? Or are there instances where a mini box really IS NEEDED despite the use of a cable card?

What is the Model # of the Pioneer TV?
What is the Make & Model # of the CableCARD?
All CableCARDs are *basically* the same. There was a time 2005-2006ish, where there were only Single-Stream CableCARDs, But all CableCARDs now are Multi-Stream CableCARDs.
Your TV is probably too old, to still be supported by Pioneer with any firmware update, that *may* be needed, but the Pioneer Support website may still have a newer firmware you could install using a USB thumb-drive.
However, Your TV should work fine with a CableCARD as long as it is Activated and "Paired" properly by Cox.
The CableCARD may have never been "Paired" to the TV. If it never was Paired you would have still been able to get any Clear QAM Locals, but once the channels are encrypted the CableCARD must be "Paired" by Cox.
The only issue will be that TVs w/ CableCARDs do not support a SDV Tuning Adapter, so any channels Cox still uses SDV with you will not be able to tune, until Cox changes them to a Linear channel, Cox has stated that all TV Starter and TV Essential Pak channels will be moved from SDV to Linear once the All Digital transition is complete.
Does the TV have a CableCARD Menu where it lists the Following:

I am guessing you have one of the following models Pioneer 6070HD / 5070HD / 4270HD

I have no idea if your TV will need a firmware update or not, but According to this posting, the firmware cannot be updated using a USB thumb-drive it takes a PCMCIA card, and Pioneer's support website does not list any available updates.

Do you have a Cox Tuning Adapter connected to your CableCARD? Some Cox markets have transitioned from the CableCARD map to the regular channel map. This means that some channels may have moved to the SDV lineup (switched digital video) and now require a Tuning Adapter. If you don't have a Tuning Adapter, you can pick one up from a Cox Solutions Store, or we can drop one in the mail to you.

There is not a single TV with a CableCARD slot that supports a Tuning Adapter, and Switched Digital Video (SDV)
TV's that could use CableCARDs were only manufactured for a very short period of time, 1 to 2 years, in 2005 to 2006'ish, and that was before the Tuning Adapter and SDV were developed and deployed. By that time the TV manufacturers had stopped supporting firmware updates, and many of those TVs did not even have a USB port to connect the Tuning Adapter.
