I am trying to get an online game working on my PC but the customer service rep for that game say I need to have certain ports open. They told me I need to contact my internet service provider about checking if these ports are open and that's what I'm trying to do.
What model router do you have? AFAIK chat doesn't help with port forwarding. Only their paid service does.

Here is a screenshot from my router settings. I don't know for sure if these are actually going through though. I'm trying to play on PC. I've played many games and I've never encountered an issue like this before, nor have I ever messed with port forwarding.

The port numbers look good but I don't see a internal IP address where you would put your PC address to forward to. Looks more like port triggering.
What is your problem with your game? Can you connect at all? NAT type? Try testing to see if the ports are open with https://www.canyouseeme.org/ Make sure the game is up and running during the test.
Also, try disabling UPnP at ADVANCED > Advanced Setup > UPnP. If it's disabled, try enabling it. On some routers UPnP bypasses port forwarding, so it's good to play with.
The internal IP address is slightly out of frame of the screenshot, I'm a bit paranoid about giving that out. I'm able to play the game in free roam but during any missions I disconnect after a couple of minutes. I'm not sure how to check my NAT type. The ports appear closed when checking on the site you linked after I input my IP address. I'm seeing if disabling UPnP fixes the issue in game now. Edit: I actually couldn't change the IP in the tool you listed and used another www.yougetsignal.com/.../Edit2: Still disconnecting in game with UPnP disabled.

Your internal IP isn't private, only your external IP. As long as it's in the same DHCP range as the router, it doesn't matter. Also, make sure to set a DHCP reservation for that IP address to the MAC address to your PC's NIC.
I would suggest trying bridge mode between the modem/router and your PC. ADVANCED > Administration > Router Mode and set to no. You may have to reboot the modem/router and PC after as this will assign a new external IP address.

It may have been removed at the firmware level. That is unfortunate. You could test the router's firewall by assigning your PC to the DMZ. The DMZ option should be available at ADVANCED > Setup > WAN Setup. This would only be a temp. work around for testing as leaving your PC IP in the DMZ is a security risk.
Assigning my PC to the DMZ has fixed all my issues but the security issue is worrying. Is there anywhere to go from here?
There is a couple things you could do. You can contact Netgear and ask them why port forwarding isn't bypassing the devices firewall. This won't fix the problem but it will report it and perhaps help others. I would start at the Netgear forum found below.
If you would rather go nuclear on the issue, you could buy a new modem and stand alone router. If you want suggestions on what to buy, just ask.
