Intermittent connection - inconsistent upload - T3 / T4 Timeouts?

I keep getting:

A: randomly disconnected (no app can access the internet for a full minute or two)

B: random ping spikes (perfect 26 ms ping then suddenly a lag spike with severe rubberbanding)

C: inconsistent upload speeds (usually a steady 11mb/s, then drop to 2mb/s or LOWER. my plan is 15mb/s up but I have trouble keeping a 2.5 mb/s live stream going)

I've been though about 3 different modems, and have had a tech come out and say everything on their side is OK. Maybe you guys can spot something in my levels/logs?
