Cox on demand via Tivo suggestion

I know there is another topic under this heading minus suggestion.  I think it would be helpful for the phone reps if there was a sheet on how the cable card and Tuning adapter info need to be setup in the customer account.  I know when I finally got ours working the rep Amber I think her name was had to do some account cleanup to get the cards paired, but forgot to check for the Tivo Ondemand code
