Constant disconnect from the Internet

This week, my Internet connection is been disconnection frequently.  Today, it already happens like every hour.  I have to power-off my router (unplug) to reacquire my Internet connection.  Can somebody check if my problem is due to my modem (I have Netgear C7000) or my lines?

    Here is my Modem Diagnostics:

    Is it your wireless connection that keeps dropping signal? I'm looking at the modem now and can see that it's online. I can see that there may be a slight issue with the upload. I'd like to clear the ARP cache and reset the modem from my end. Please reach back out here with a good time to take care of that.
