Audio and Video often out of sync

At any given time, I can browse stations and find several where the audio and video are out of sync.  It's random channels and programs, it's not just the same channel or program every time.  And it's usually just the program itself, when commercials play, the audio and video is in sync.  This seems to prove it's not my equipment or signal.  If I search these forums for this issue, I find that it's common.  Can there be some sort of universal solution to this?  This is very annoying.
