There are 1,000s of posts about Cox & Fortnite(in these forums, fortnite forums, and reddit). Cox keeps sending people and charging $75 for this exact issue. I had someone come and my internet was "the cleanest I've ever seen" from the technicianand said the issue is not on their end and left. All I know is that it started happening about a month ago. Speedify VPN works because it essentially gives you the direct hop to Speedify's server which is closer to the fortniteservers and have different hops between servers. I can use VPN's and get 0 packet loss, but it still feels laggy. Just look up other ISPs in your neighborhood and leave cox if you have that option! I don't even have another option in my neighborhood. I pay for Gigablast Cox and my friends are ~1 mile away on Spectrum and get 0 loss on the same games as me. I just ran the traceroutes on the same IPs from the ones in this thread. Last night, I played for about two hours and had two to three games with packet loss which is the opposite of what happened prior. I'm not sure how to get the IP of the server I'm experiencing packet loss on.
